Introduction to Civil Registration
1.Issuing of identity cardsIssuing of identity cards.
2.Issuing Birth, Death, Marriage Certificates
3.Past births (births after three months) Registration.
4.Registration of past deaths (three months after death).
5.Expiration of the name of the birth certificate (below 21 years, over 21 years) / Change (below 21 years, over 21 years)
6.Changing / inserting / abandoning a birth certificate (mother's information / father's information).
7.Registration of marriages done in the registered churches in the division.
8.Registration of marriages within offices.
Amending the error in the mother's name on the birth certificate
- Original birth certificate to be corrected
- Original birth certificate of mother
- Copy of mother's national identity card
- Original copy of the parents' certificate of marriage
Include or rectify the field of father's name Grand Father's Name.Civil Stauts of a Birth Certificate
- The original copy of the birth certificate which can be corrected
- Original birth certificate of father
- Original copy of the parents' certificate of marriage
- Mother/father's National Identity card copy
Registration of the Past Birth
- Please bring below documents to the District Registrar of the Department with Parents and child.
Birth Description (Original copy). - Photo vaccine card copy.
- If the parent is married, the marriage certificate of the marriage certificate.Original Birth Certificates of Parents.
- Photocopies of Mother and Father National Identity Cards.
- If the birth is less than 1 year, stamps of Rs. 50.
- 100 stamps if more than 1 year.
Please take the above documents and come to the District Registrar's Office with the mother and father child.
Registration of the Past Death
- Please bring below documents to the District Registrar of the Department.
- Original death certificate of the hospitalif died in a hospital.
- Certificate of Grama Niladhari.
- Condemnation / death ads.
- Cash receipt of coffin.
- Letter from the Chief Incumbent of the temple.
- If the death is less than 1 year, if the stamp is more than Rs. 50 / sterling over 1 stamp.
- Birth certificate of the person / marriage certificate and a copy of the National Identity Card.
Please take the above documents and come to the District Registrar's Division on Monday or Wednesday.
